Abstract

Thyroid hormone abnormalities known as nonthyroidal illness syndrome (NTIS) are characterized by a fall in serum triiodothyronine levels in total and free forms (TT3 and FT3) and an increase in the reverse form (rT3), concurrently with normal thyrotropin (TSH) levels. A more severe underlining disease can result in a decrease in serum thyroxine (T4). It occurs mainly in patients with acute and severe diseases, particularly in those admitted in intensive care units. The physiopathology involves diminished peripheral conversion of T4 to T3 and inactivation of T4 to rT3 resulting in lower T3 levels and increased rT3. There is no elevation in TSH, as would be expected. It has been established an important correlation between worse prognosis and lower serum T4 levels. Much has been studied about this syndrome in severe diseases, but not only in acute settings. The syndrome has been described in patients with chronic conditions under outpatient care, including Diabetes Mellitus, mostly when presenting with poor glycaemic control. However, there is a lack of studies relating the presence of its complications (nephropathy, neuropathy and retinopathy) and subclinical systemic inflammation with the severity and evolution of NTIS, as well as its thyroid hormone profile.
