Cost-efficacy and adolescents' acceptability in treating initial approximal caries lesions using diammine silver fluoride: a randomized controlled clinical trial

Abstract

The present study aims to evaluate the effectiveness of 30% Silver Diamine Fluoride (SDF) in the treatment of initial caries lesions in approximal surfaces of premolars and permanent molars compared to the treatment with resin infiltration and local biofilm control by flossing. For this comparison, 198 teeth of 12-to-17-year-old adolescentes will be selected. They are supposed to have at least one approximal surface with clinically caries lesion into enamel, presenting or not radiographic images. If have any radiographic change, this one should be restricted to enamel or outer third of dentin. Individuals will be randomly allocated according to the treatment used: (I) SDF, (II) resin infiltration, (III) instruction for daily flossing by the individual. All adolescents will receive the treatment to which they were allocated to and also the placebo treatments corresponding to other groups. The treated teeth will be evaluated after 1 and 18 months regarding the presence of biofilm and compliance with daily flossing. The treated approximal surfaces will be evaluated after 6, 12 and 18 months regarding the progression of caries lesions through visual and tactile examinations using a ballpoint probe. Radiographic examination will be performed at baseline and after 12 and 18 months. For radiographic examination, pairwise evaluation and radiographic subtraction will be performed. Multilevel statistical analyses will be carried out to verify the most effective treatment for controlling the initial caries lesions on approximal surfaces of permanent posterior teeth, considering the progression of caries according to the chosen outcomes and also to investigate possible associations between social and biological factors with the effectiveness of such methods.
