Abstract

Magnetic resonance imaging (MRI) is an important non-invasive tool for the evaluation of several conditions. In drug-resistant epilepsies, surgical outcome depends largely on complete removal of the epileptogenic region, making MRI crucial for surgical planning. Hippocampal sclerosis and cortical dysplasia are the most common pathological surgical findings in these drug-resistant patients, and are detected in MRI as increased T2 signal, reduced magnetization transfer and increased mean water diffusion. However, most cortical dysplasias remain undetected by the most modern MRI protocols, and data from our previous studies indicate that even hippocampal sclerosis may not be detected always. Consequently, a significant percentage of patients fail to achieve postsurgical seizure control. Thereby, it is important to optimize MRI to detect more subtle pathological changes, as well as to define the pathological substrates of the MR signals. Our aim is to identify, by proteomics, the histologic substrates of the different MRI protocols, and to improve the detection of cortical dysplasias by quantitative MRI. (AU)
