Evaluation of cardiac autonomic modulation and functional capacity in the overlap of Chronic Obstructive Pulmonary Disease and Obstructive Sleep Apnea Syndrome: a follow-up study

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a systemic and pulmonary disease that, in association with other diseases, such as Obstructive Sleep Apnea Syndrome (OSAS), may cause greater impairment of functional performance and quality of life. However, it is little known about the coexistence of COPD and OSAS and its impact on the autonomic cardiovascular system and functional impairment in the follow-up over a year. Objectives: To evaluate and determine the impact of OSAS overlapping on COPD, following a year of follow - up, in cardiac autonomic behavior during active and passive postural changes, respiratory sinus arrhythmia maneuver and exercise, as well as functional capacity. Methods: in the first stage, volunteers aged 50 and over will perform a clinical evaluation, pulmonary function tests, echocardiography and sleep home examination in order to be allocated to COPD or COPD + OSAS groups. In the second stage, they will respond to questionnaires and perform an assessment of body composition through bioimpedance and, in the third stage, will pass through the evaluation of autonomic behavior through Heart Rate Variability (HRV) at rest during passive and postural changes active, in the respiratory sinus arrhythmia maneuver, and during the six-minute walk test. In this last test will also be evaluated the functional capacity of the volunteer by means of distance traveled. After a period of one year, in the fourth stage, these patients will be reassessed regarding the aspect of HRV (rest, passive postural change, respiratory sinus arrhythmia maneuver and exercise) and functional capacity. Expected results: It is expected that cardiac and functional autonomic alterations are more impaired in the COPD group with OSAS overlapping in relation to the isolated COPD group, both at rest and during exercise, and that there is a more pronounced functional and autonomic impairment according to progression and severity of diseases.
