Microscopic analysis of the genesis and progression of periapical lesions after systemic and local use of probiotic in mice

Abstract

Probiotics are living microorganisms that when administered in adequate amounts confer benefits to the health of the host. Although the research on the use of probiotics in the medical field is more established, their use for the treatment of diseases in the oral cavity has promising perspectives, in which some dental specialties already use it in in vivo experiments, evidencing inhibition of inflammatory bone loss and alteration in the systemic immune modulation. However, few studies have reported the effect of probiotic use on pulp infections and none have used the same as an irrigation solution. Thus, the proposal of this study will be to evaluate the effect of probiotics when consumed in water and used as irrigating solution of root canals of rat teeth with induced periapical lesion. The animals will be divided into ten groups, as follows: Group I (4 animals/8 teeth): Water without probiotics, without periapical lesion (3 days); Group II (4 animals/8 teeth): Water with probiotics, without periapical lesion (3 days); Group III (12 animals/24 teeth): Water without probiotics, with periapical lesion (7, 21, 42 days); Group IV (24 animals/48 teeth): Water with probiotics, with periapical lesion (7.21,42 days); Group V (4 animals/8 teeth): Water without probiotics, 2.5% sodium hypochlorite as an irrigating solution, without periapical lesion (3 days); Group VI (4 animals/8 teeth): Water without probiotics, probiotic as an irrigating solution, without periapical lesion (3 days); Group VII (24 animals/48 teeth): Water without probiotics; 2.5% sodium hypochlorite as an irrigating solution, with periapical lesion (7, 21, 42 days); Group VIII (24 animals/48 teeth): Water without probiotic; probiotic as an irrigating solution, with periapical lesion (7, 21, 42 days); Group IX (24 animals/48 teeth): Water with probiotic; probiotic as an irrigating solution; without periapical lesion; Group X (24 animals/48 teeth): Water with probiotic; probiotic as an irrigation solution, with periapical lesion (7, 21, 42 days). In groups where the animals will receive the oral probiotic (water consumed by the animals), the probiotic Bifidobacterium animalis subsp. Lactis (B. lacti) HNO9 will be added to potable water in a volume of 10 milliliters, in the proportion of 1.5 x 108 colony forming units (CFU) per milliliters. In the groups VI, VIII, IX and X, the probiotic is administered as irrigating solution formulated by adding the probiotic in an aqueous medium containing 1.9 x 109 CFU of probiotic same used orally in a final volume of 1mL. The root canals will be irrigated on day 0 and days 3, 7, 14, 21 and 28 days of experiment. The samples stained in HE in optical microscopic will be analyzed for the description of the characteristics of the pulp tissue and apical and periapical regions. Also, morphometric analysis of periapical lesions area, under fluorescent microscopy, will be performed. Indeed, osteoclast labeling, histoenzymology for tartrate-resistant acid phosphatase (TRAP), and immunohistochemical activity for identification of osteoclastogenesis markers and macrophages M1 and M2 subtypes (RANK, RANKL, OPG, M1-CD68, NOS2,M2-CD163, Arg-1) will be performed, in addition to the microbiological analysis by the Brown & Brenn technique. All analyzes and graphical representations will be performed with GraphPad Prism 7 software (GraphPad Software Inc, San Diego CA, USA). The statistical tests applied will be selected according to the nature of the data. For all analyzes, the significance level of 5% will be adopted. (AU)
