Application of magnetic resonance imaging texture analysis as a new tool for the study of temporomandibular joint disc changes in migraine headache patients

Abstract

The aim of this study will be to use the texture analysis (TA) technique in magnetic resonance imaging (MRI) as a tool to identify possible changes in the articular disc of the temporomandibular joint (TMJ) of individuals with migraine-type headache (IM), related to internal derangements and comparing the values provided by this technique with those obtained in a control group of individuals without migraine headache (control group). 80 MRI scans of the TMJ will be selected, being 40 scans of individuals without MI or any type of headache (control group) and 40 MRI scans of individuals diagnosed with MI (study group), according to the criteria of the International Classification of Headache Disorders (2004). All MRI scans were acquired using the same protocol, using a 8.0 cm diameter bilateral surface coil, with T1 and T2-weighted, lateromedial parasagittal images, with closed mouth and maximum mouth opening. The T1 images will be analyzed, looking for the internal derangements, considering the position and function of the articular disc. For TA, closed-mouth T2-weighted MRI images will be evaluated, and 03 consecutive parasagittal sections will be selected, and, through the software MaZda 3.20 (Institute of Electronics, Technical University of Lodz, Poland), the articular disc will be segmented, with the Draw Polygon tool, in order to determine the region of interest (ROI) where the TA will be performed. The texture parameters will then be calculated using the co-occurrence matrix. The results will be tabulated and, for each TA parameter obtained, will be made an average of the three sections analyzed (lateral, central and medial). The data will be submitted to the Shapiro-Wilks test to assess normality. Subsequently, the T-test (if the distribution is normal) will be used to study between the values derived from the TA and those related to the analysis of the internal derangements (disc displacement and disc function), or by the Mann- Whitney Rank Sun if the distribution is not normal.
