Infections associated with orthopedic implants: microbiological analysis of the sonicate fluid in blood culture bottles and in conventional culture

Abstract

The number of arthroplasties and osteosynthesis has grown rapidly and tends to increase even more in the next years. Consequently, the absolute number of infections due to these procedures has also grown, which has encouraged countless studies to search for the best technique to diagnose implant infection. The development of the technique of implant sonication with inoculation of the sonicate fluid in blood culture bottles has presented excellent results in most of the studies in regard to sensitivity, specificity, and culture time. Thereby, our study seeks to make a comparison of accuracy, efficiency, and quickness between the techniques of sonicate fluid in blood culture bottles and conventional sonicate fluid culture. Complementarily, the implants, etiological agents, antimicrobial susceptibility profile, and demographic aspects of the patients will be studied. The project will be executed between August 2021 and July 2022, and the articular prosthesis and internal fixation devices will be removed during revision and resection surgeries performed in the Department of Orthopedics and Traumatology at Hospital da Santa Casa de São Paulo. The implants will be taken to the laboratory of Faculdade de Ciências Médicas da Santa Casa de São Paulo and submitted for sonication in order to inoculate the sonicate fluid in blood culture bottles and conventional media culture. The criteria of the studies from the European Bone and Joint Infection Society and W J Metsemakers et al. will be used to define infection associated with articular prosthesis and fracture, respectively.
