Predictive value of cardiovascular disease secondary to Chagas Disease on clinical course of patients hospitalized as a consequence of COVID-19

Abstract

Background: Chagas Disease (ChD) is associated with several cardiac, cerebrovascular, and thromboembolic complications. Impressively, recent studies have shown similar associations with Coronavirus Disease 2019 (COVID-19). Objective: Assess the additional impact of ChD on the clinical course of patients hospitalized as a result of COVID-19 compared to non-Chagasic patients. Material and Method: longitudinal, observational and retrospective study, considering patients with heart disease secondary to ChD consecutively admitted consequently to COVID-19, from March 2020 to March 2022. In addition to matching for sex, age and comorbidities, the control serie will be stipulated based on the risk ratio for mortality in patients with ChD, seeking to guarantee the study a power of 80%. Demographic profile, comorbidities, complementary exams, therapeutic strategies, and adverse events will be evaluated in the short and long term. The primary outcome will include death from all causes and heart failure decompensation. Secondary outcomes will comprise: cardiovascular death, thromboembolic phenomena, arrhythmias requiring intervention, decline in renal function, need for oxygen supplementation and/or use of ventilatory support length of hospital stay, and manifestations compatible with long-term COVID-19 syndrome. Cox proportional hazards analyzes will be used to determine the predictive value of ChD in the occurrence of outcomes. Kaplan Meier curves will be constructed to demonstrated event-free survival of groups. P value < 0.05 will be considered as statistically significant.
