Abstract

Oral leukoplakia (OL) and proliferative verrucous leukoplakia (PVL) are oral potentially malignant disorders (OPMD) that require surgical treatment when presenting with epithelial dysplasia. However, the recurrence and malignancy rates of OL and PVL persist high and remain a clinical challenge for patients and professionals involved in the treatment. Moreover, currently, the use of artificial intelligence (AI), through prognostic models, is an important method to auxiliary predict the risk of OPMD malignancy. Therefore, this study aims to evaluate the performance of surgical treatment by comparing two modalities (diode laser and scalpel) in reducing the rates of recurrence and malignancy of OL and PVL, as well as, to analyze the performance of a predictive model via deep learning neural networks for recurrence and malignancy of OL and PVL after treatment. This study is designated as an adjunctive randomized clinical trial to an AI study. One hundred and twenty patients diagnosed with OL and PVL will be randomized into 4 treatment arms: (1) OL treated with a diode laser (n=30); (2) OL treated by scalpel (n=30); (3) PVL treated with a diode laser (n=30) and (4) PVL treated by scalpel (n=30). Follow-up of patients recruited and enrolled in the clinical study will be performed at a three-monthly frequency for 24 months for clinical evaluation of the clinical outcomes of the interventions. Based on deep learning neural networks, sociodemographic data, clinical and histopathological photos of the clinical trial patients will be used to investigate the clinical outcomes. It is expected that the diode laser will show a decrease in recurrence and malignancy rates of OL and PVL when compared to the scalpel, and AI may help predict the risk of recurrence and malignancy of OL and PVL
