Risk and vulnerability to oral diseases in Brasilian adolescents

Abstract

This cross-sectional study of quantitative and qualitative nature aimed to evaluate the risk of oral diseases and vulnerability in adolescent 15-19 years in Piracicaba - SP. The information risk caries, DMFT and periodontal disease were collected and the socioeconomic, behavioral, demographic and psychosocial variables were recorded by means of the OIDP, WHOQOL, and socioeconomic semistructured questionnaire. In addition, in-depth interviews were conducted with adolescents in need of treatment after 12 months of diagnosis. A random sample of 1179 adolescents from 21 public schools in the areas of 34 Family Health Units (USF). The oral examination was performed with CPI periodontal probe and dental mirror under artificial light by two trained examiners. To evaluate the association between clinical, socioeconomic, behavioral, demographic and psychosocial, univariate analyzes were performed using the chi- square test and Fisher's exact test and then, multilevel analyzes to identify risk variables. In the qualitative phase, interviews were transcribed, key data grouped by categories and themes identified allowing the thematic analysis. The results in caries and DMFT dependent variable, teens who needed dentures, have declared convict in the family and living in houses with more people, had more decayed teeth. There were a greater number of decayed teeth and DMFT with worse self-perception of oral health. Being female, having already had toothache, age and time since last dental visit were related to DMFT. With regard to contextual variables, the DMFT was lower in the suburbs with greater access to domestic sewage and the number of decayed teeth was higher in neighborhoods with the worst levels of social exclusion. For the dependent variable periodontics, teens who have never had a toothache, with a lesser impact on oral health and living in houses with higher percentage of domestic sewage exhibited less periodontal disease. For the dependent variable and OIDP WOQUOL, the girls had worse quality of life and greater impact dental partner. There was a decrease in quality of life, and increased with increasing OIDP periodontal index. In turn, there has been an increase in quality of life and reduction OIDP with increasing income. Adolescents residing in areas of greater social exclusion and greater impact dental partner had a poorer quality of life. For qualitative research, nonadherence to treatment was related to some aspects and not grouped in priority, priority and priority change. We conclude that individual and contextual variables were associated with the presence of caries, DMFT, periodontal disease, impact and quality of life in adolescents, indicating that it should be taken into consideration in the formulation of policies for the promotion of oral health and activities prevention and that the main reasons for non-adherence are related to different priorities and braces as a motivator priority to oral health among adolescents (AU)
