Aspects related to dental loss in adults and with edentulism in the elderly: cross-sectional study

Abstract

Dental losses and edentulism present a serious public health problem. This work is a master's thesis in the form of two articles, where the first one was intended to describe the profile of dental losses in adults and the second the epidemiology of the need for total dental prosthesis in the elderly population through the SB Paulo 2015. The objective of this study is to analyze the dental losses in adults, to evaluate the amount and position of these teeth lost in the population, to analyze the progression of dental losses to edentulism in the elderly population, to verify the factors associated with the use of total dentures in the elderly. A cross-sectional, statewide study with representation for adults (35-44 years) and elderly (65-74 years) living in the State of São Paulo. Data from this study were taken from the epidemiological survey of oral health in the State of São Paulo (SBSP 2015). A total of 5.9% (n = 359) of the adults examined presented loss of 1 to 4 first molars, 25.3% (n = 1530) lost up to 12 posterior teeth, 32.7% (n = 1977) lost up to 12 teeth, including one / more anterior, 9.4% (n = 568) lost 13-31 teeth, and 1.9% (n = 117), were total edentulous. Women presented higher ratio of prevalence of tooth loss increased from 1.19 to 1.42, or 19 to 42%, in adults in the state of São Paulo, among those who lost up to 12 teeth including anterior and above 12 teeth . The lowest income varied the prevalence ratio of dental loss increased from 1.78 to 1.97 between 1500 and 2500 reais, and from 1.39 to 1.65 below 1500 reais. The shorter schooling was associated with dental loss between 5 and 9 years of schooling, ranged from 1.39 to 4.71 and increased the number of teeth lost. In article 2, in the elderly population, those who considered themselves unhappy presented 2.5 times more chances of needing double PT, as well as the elderly with oral health impact in daily life (OR = 1.9), dissatisfied with oral health (OR = 5.7), last consultation due to pain (OR = 3.1) and in public service users (OR = 1.4). Older individuals with years of schooling and number of assets below the median also had a greater chance of double PT. The results point to the determinants of dental losses and the need for total prosthesis in several dimensions, reflecting both the influence of socioeconomic aspects and elements of social capital, self - perception in health and access to dental services. Measures that act in a multidimensional manner are necessary for the repair of this historical heritage of (dis) dental care throughout adult life and worsening in the elderly population (AU)
