Impact of atraumatic restaurative treatment associated with oral health educational strategy on oral health-related quality of life of schoolchildren

Abstract

This study aimed to evaluate the impact of atraumatic restautaive treatmeant (ART) associated with oral health educational strategy (OHES) on oral health-related to quality of life (OHRQoL) of schoolchildren. A randomized clinical longitudinal study was performed with a population-based sample of 124 six- to eight-year-old students, recruited from public schools, allocated in two groups according to the presence (OHES group ¿ OHESG, n=62) or not of dental caries. The group with dental caries was subdivided according to type of dental procedure, resulting in: OHES+ART group (OHES+ARTG) (n=31) and ART group (ARTG) (n=31). OHES consisted of theoretical and practical activities once a week during four weeks. ART was performed using hand instruments (ART Kit Duflex® ¿ SS White) and a high-viscosity glass-ionomer (Ketac Molar Easymix® ¿ 3M ESPE). OHRQoL was assessed by short-form of Child Perceptions Questionnaire (CPQ8-10-ISF:16), global ratings of oral health (OH) and overall well-being (OWB) and global transition of change in oral health. The evaluations were performed in the following periods: baseline ¿ before OHES (OHESG and OHES+ARTG) or ART (ARTG), one week after OHES (OHESG and OHES+ARTG) or ART (OHES+ARTG and ARTG); one month after OHES (OHESG) or ART (OHES+ARTG and ARTG). Data were analysed using Shapiro-Wilk test, Chi-square independence/partition or Fisher¿s Exact tests, Mann-Whitney or Kruskal Wallis tests, Friedman (Tukey post-test), with a 5% significance level. Only in OHESG, the frequency of biofilm score 4 and gingivitis score 2 decreased one week (p=0.003) and one month (p=0.033) after OHES, respectively. OHES+ARTG presented lower CPQ8-10-ISF:16 total (p<0.0001) and oral symptoms (p=0.0003) scores one week and one month after ART; as well as, lower functional limitations scores (p=0.039) one week after ART compared to baseline. OHESG had lower CPQ8-10-ISF:16 total score at baseline than the other groups (p=0.042). More than half of OHES+ARTG (58.06%) rated their OH, in general, `slightly changed¿ one week after OHES (p=0.049), while 70.97% and 64.32% considered it `much changed¿ one week (p=0.001) and one month (p=0.009) after ART, respectively. In relation to OWB, 53.23% of OHESG rated it `much changed¿ one month after OHES in constrast with 67.74% and 64.52% of OHES+ARTG one week (p=0.001) and one month (p=0.007) after ART. All groups reported improvement in oral health perception, however it was more frequently reported by children with dental caries and after ART (p<0.0001). Children who rated their OH as `excellent¿ had lower CPQ8-10-ISF:16 total scores than the other categories, in all periods for OHESG (p<0.01) and one week after ART for OHES+ARTG (p<0.05). ART associated with OHES positively impacted on OHRQoL, especially in reduction of oral symptoms and functional limitations, of children with dental caries (AU)
