Distorted thoughts and avoidance behaviors during depressive episode: a longitudinal study

Abstract

Background: depression is a highly prevalent mood disorder bearing negative consequences for individuals in psychologically, socially and physically domains. Although dysfunctional thoughts are usually described as indicators of severity and poor response in depression, there are few studies in this area using instruments specifically designed for this purpose. Aim: to investigate the presence of dysfunctional thoughts and avoidance behavior in depressive patients throughout the course of a 6-month drug treatment, without psychological intervention. Methods: 126 patients with a mean age of 40 years (SD= 11), predominantly women (n=90; 68.7%) diagnosed with Major Depressive Disorder (without any comorbidity) by psychiatrists and confirmed by the structured Interview for DSM-IV (SCID-CV). Hamilton Rating Scale Depression-HAMD, Montgomery-Asberg Depression Rating Scale (HAMD > 14) was included. Patients were recruited from a tertiary care center and followed up for 6 (six) months and periodically evaluated by the HAMD, Montgomery-Asberg Depression Rating Scale (MADRS), Cognitive Behavioral Avoidance Scale (CBAS), Depressive Thoughts Scale (EPD) and Beck Depression Scale (BDI II). Results: the severity of depressive symptoms (HAMD and MADRS) decreased significantly at month 2, and from this moment on, the reduction of symptoms was shown to be slower; depressive thoughts and avoidance (EPD and CBAS) also indicated a significantly decreased in the first weeks, however, did not continue in the same linear reduction as evaluative scales of symptoms, indicating that, regardless of the depressive symptoms decreased and the patient return to do their daily activities, their thoughts remain distorted. Conclusions: results show a substantial improvement in behavioral avoidance patterns and negative thoughts in the patients treated, even with the administration of antidepressant medication only. However, these changes were not significant enough to provide a remission of these symptoms, reinforcing the importance of studies focused on the functional recovery of the patient. Also, results lead one to question if depressive thoughts can be interpreted as traits or condition, as well as the need for an investigation that also considers possible genetic printings (AU)
