Behavioral Analythical Therapy for anxiety and depression of women: elaboration, aplication and evaluation of semi-structured procedure (Promove – Saúde da Mulher)

Abstract

The prevalence of anxiety and depression disorders is a Brazilian public health problem. In women, this prevalence is even higher. Most of the cases show comorbidity between the two disorders, what aggravates the functional impairments. Behavioral-Analytical Therapy is one of the possible treatments, but it lacks scientific evidence. The present study proposed a semi-structured intervention with this approach for three participants, women who worked, had marital relationships and children, and were identified by reports and instruments as having anxiety and depression. The objectives were to describe and analyze, in three clinical cases, the application and effects of Promove – Saúde da Mulher, designed to reduce anxiety and depression symptoms, through the development of specific behavioral repertoires, especially social skills. Study I describes and compares the clinical case formulations, elaborated through two methods, for the three participants. Study II describes and compares, for the three cases, the effects of Promove – Saúde da Mulher, on the acquisition of repertoire and its impact on the reduction of complaints and symptoms of anxiety and depression and, also, participants satisfaction with the intervention. The design used was an adaptation of multiple baseline among participants and adopted the following materials: GAD-7, PHQ-9, IHS-2 Del Prette, personalized complaints Checklist, evaluation of the therapeutic process (written survey) and reports prepared after the sessions. Standardized instruments’ results were analyzed statistically through JT Method. Study I results indicated common risk factors for the disorders and individual factors. Study II indicated anxiety and depression symptoms reduction of the three participants to non-clinical levels, except for the depression level of one participant. Various statistically reliable changes occurred during the intervention. There was a methodological difficulty in detecting the expansion of the participants' behavioral repertoire, although some results point to this outcome. The three participants were satisfied with the intervention, according to the written record obtained by the written survey. There was also an important reduction in the number of clinical complaints throughout the intervention. This study contributes to the production of evidence in Analytical-Behavioral Therapy and describes the scope of the intervention, which can be replicated and improved in future studies. (AU)
