La clinique de l'urgence et le prélude aux entretiens préliminaires : une praxis entre la lutte des classes et les formations de l'inconscient

Abstract

This thesis has in its outcome a work of eight years of intercession-research, whose theme is the Emergency Clinic in a hospital unit. The intercession-research derived from the doctoral thesis presented is the result of a work in a “Santa Casa de Misericórdia”, one of the hospitals of a city in the interior of the state of São Paulo. The psychologist cautioned by Freud and Lacan's Psychoanalysis and by Marx was the position taken, during the experience close to the subjects of suffering and to the collective of workers, particularly to the Intercessor Device and the worker-intercessor. Mobilized by the practice of urgencies, our questions revolve around the following question: is it possible for an Urgency Clinic that, besides offering analytical listening to the psychic impasses, in equal content, wants to intercede in the fundamental division of work in favor of the Hospital Health teams? We maintain that the ER Clinic in a hospital unit is only effective if we also aim at analytical listening to the subjects of suffering and the intercession in the institutional establishment, together with their modes of health production and subjectivity. The Intercessor Device was the "method" instrument that we used for the clinical positions in the hospital establishment and the subsequent writing of the reflection in the form of a doctoral thesis. It is less a traditional method of research than a way of operating in praxis, it appropriates elements of Freud and Lacan's Psychoanalysis, the Marxian logical-category complex, Lourau and Lapassade's Institutional Analysis and the Philosophy of Difference (especially with the Deleuzian concept of "intercessors"). We defend that from the point of view of an Ethics of the singularization of desires, averse to the production of health as a serialization of movements of life in society, listening to the subject of the unconscious implies not covering the eyes in front of the instituted models that reiterate the modes of capitalistic sociability in the Health institution. In this way, we expand the concept of Emergency Clinic, based on Psychoanalysis, linking it, in a certain sense, to Marx's thought, with respect to the Brazilian reality and to the ethical-political bases of the Sanitary Reform, a revolutionary movement of health practice that gave rise to the SUS - Unified Health System.
