Abstract

This pioneering study investigated the feasibility and acceptability of a supervised six-month physical training program for patients with severe systemic lupus erythematosus (SLE) following glucocorticoid (GCs) pulse therapy. Four metrics were evaluated: (A) recruitment capacity, (B) acceptability and satisfaction, (C) resource requirements, and (D) intervention efficacy.Recruitment capacity encompassed the effectiveness of the strategies employed, the time required, recruitment rates, and the eligibility criteria of the participants. Acceptability and satisfaction analyzed the acceptance of the physical training program, including motivators and barriers to exercise, adherence, retention rates, engagement, satisfaction, safety, and reasons for discontinuation. Resource requirements assessed the need for personnel, time, and financial costs involved in implementing the program. Finally, intervention efficacy measured the impact of physical training on the participants\' musculoskeletal health, aerobic capacity, strength, and functional capacity.The participants faced various daily challenges stemming from the disease and its treatment, reflected in the complexity and low adherence to the proposed exercise program. Although they recognize the health benefits of exercise and report significant motivations, such as improved quality of life, substantial barriers related to physical health, emotional well-being, and socioeconomic conditions persist. The presence of specialized physical education professionals is crucial to adapt programs to individual needs and provide continuous support.This study highlights the need for adaptive and personalized strategies to gradually integrate exercise programs into the routines of these patients, considering the unique complexities they face (AU)
