The Role of 3D printing in the positioning of the Glenoidal Component in Total Shoulder Arthroplasty

Abstract

Shoulder osteoarthrosis is a frequent clinical condition in patients mainly after the sixth decade of life leading to functional limitation and disabling pain. A wear occurs in the glenohumeral joint, compromising the joint between the humerus and the scapula. Total shoulder arthroplasty is the surgical treatment of choice when there is no improvement with conservative treatment. The key point of this procedure is the correct implantation of the glenoid component, which is difficult to perform due to the anatomy of the scapula, which can lead to improperly positioned implants, resulting in component loosening and other complications.The objective of the study was to verify the accuracy of the use of 3D prototyping (Intervention Group - GI) in the operative planning for the positioning of the glenoidal component, compared to the conventional planning technique with the use of tomographic sections (Control Group - GC).Based on 10 tomographic examinations of patients with osteoarthrosis in the glenohumeral joint, 11 scapulae from each examination will be printed, totaling 110 anatomical prototypes. Ten prototypes of each exam (100 prototypes) will undergo surgical implantation of the glenoidal component. Each of the five surgeons will perform 20 procedures (10 GC deployments and 10 GI deployments), twice in each of 10 printed tomographic models. A prototype of each CT will be used as a guide in the planning of the GI by all surgeons. The prototypes will be made of polylactic acid (PLA) in a 3D printer.The implantation of the glenoidal component in the prototypes in each of the 10 cases of osteoarthrosis will occur in both GC and GI groups. The glenoidal components will be implanted in the prototypes using two planning methods. A method based on tomographic cuts of each case and using the goniometer (GC), and another method based on the 3D vision of the prototyped 3D anatomical model of the affected scapula, at the moment of the implantation of the glenoid component of the corresponding case (GI).As a result, the failure and positioning of the glenoid component will be evaluated.
