Abstract

Antimicrobial resistance (AMR) has been considered one of the main threats to the human race in the 21st century. We are facing an unstoppable fast-forward instance of Darwinian adaptation through natural selection, in which microbes are constantly evolving to survive antimicrobials. As part of their natural behavior, they adapt continuously. They will succeed unless we develop effective approaches to control and prevent further AMR spread. We propose to establish the Antimicrobial Resistance Institute of São Paulo (ARIES) that will join researchers from 5 universities in São Paulo with experts from public organizations (EMBRAPA, CETESB) and representatives of the Ministry of Agriculture, Livestock and Food Supply, Ministry of Health, Brazilian Health Regulatory Agency (ANVISA) and Pan American Health Organization (PAHO). Working together in a pioneering enterprise, they will fight against AMR implementing the concepts of Global Health and One Health. Mitigation of the AMR threat requires a multidisciplinary approach that recognizes and interferes in the drivers of AMR in the human-animal-environment interface. ARIES will count on five main pillars to achieve our key objectives: 1) Socio-Environmental Observatory and Data Science Center: surveillance of ecosystems involved in the emergence and spread of AMR; 2) Characterization of microbial evolution, mechanisms of AMR and micro-organism fitness and pathogenicity; 3) Development and validation of new strategies for control and prevention of AMR in humans, animals, and environment; 4) Translation of the successful initiatives to Business and Public Health Policies; and 5) Diffusion of ARIES - Knowledge in connection with Society, Education and Dissemination. ARIES will not only contribute to elaborate a more sophisticated and comprehensive surveillance system able to early detect the emergence of AMR in the context of One Health, but also generate new strategies to respond, contain, and prevent the resistant infections across healthcare settings, food, and communities.
