Performance evaluation of the fluorescence devices to detect caries lesion: in vivo and in vitro study

Abstract

The aim of this study will be to evaluate the in vitro and in vivo performance of the light-induced fluorescence methods (DD, DDpen e VistaProof), visual examination (ICDAS II) and radiographic examination for smooth and occlusal surfaces caries detection. For that purpose three researches will be performed. For the purpose I, it will be analyzed the performance of the DD (KaVo, Germany), the DDpen (KaVo, Germany) and the VistaProof intra-oral camera (Dürr Dental, Germany) for monitoring the caries lesions development in 120 enamel blocks from bovine incisors kept frozen at -20oC . First, the superficial microhardness analysis will be performed (Metaserv 2000 - Buehler, USA), followed by the examinations described above. After that, a protocol for artificial caries induction using a bacteriological model will be applied. The caries lesions obtained will be again analyzed using the same fluorescence methods and a new superficial microhardness analysis will be performed. For the purpose II, it will be evaluated: (a) the in vivo performance of the light-induced fluorescence methods (DD, DDpen e VistaProof) comparing with the ICDAS II criteria for occlusal caries detection; (b) the in vitro performance of the methods describe above and (c) the validation of the in vivo results comparing with the in vitro results. It will be selected 100 human erupted (molars and premolars) which must present sound or carious occlusal surface. In the in vivo analysis, the measurements will be performed by one examiner using the fluorescence methods (DD, DDpen e VistaProof) and the visual criteria ICDAS II. After teeth extraction, two examiners will perform the analysis twice, with a one week interval, using the same methods described. For the purpose III, it will be evaluated the in vitro reproducibility and accuracy of the light-induced fluorescence methods DD (KaVo, Germany), DDpen (KaVo, Germany) and VistaProof (Dürr Dental, Germany), ICDAS II criteria and digital radiographic examination for occlusal caries detection in 100 exfoliated deciduous molars. The teeth must present sound or carious occlusal surface. Two calibrated examiners will perform the analysis twice, with a one week interval, using the same methods described above. For the purposes II and III, the histological examination will be performed for the validation for caries extension. Sensitivity, specificity, accuracy and area under the ROC curve will be calculated for each method at D1, D2 and D3 thresholds. The McNemar test (p<0.05) will be used to compare those values. For the reproducibility inter- and intra-examiner the ICC and the unweighted Kappa coefficient will be calculated. (AU)
