Abstract

Fibromyalgia is a painful syndrome characterized by chronic diffuse pain and involvement of multiple muscles and soft tissues.Its complex and diffuse nature suggests that neurological and psychological mechanisms, including decreased pain threshold, are involved and that the change of sleep can contribute to the development of the syndrome. Other changes include disturbances in pain modulation in peripheral and central levelsThe pathophysiological mechanisms of fibromyalgia are related to changes in neurotransmitters. Cytokines may be involved in modulation of symptoms such as hyperalgesia, fatigue and depression, and maintenance of sympathetic pain. The substance P (SP) is a neurotransmitter stored in nociceptive afferent fibers and appears to be involved in the pathogenesis of musculoskeletal pain, primarily causing neurogenic inflammation. The level of SP is increased in cerebrospinal fluid of fibromyalgia patients compared to control groups.The infusion of intravenous anesthetic is indicated for fibromyalgia when systemic drugs do not produce adequate effect. Intravenous lidocaine promotes sympathetic blockade, vasodilation, anesthetizing the nerve endings in the vascular endothelium, stabilize membrane and break the circle vicious that keeps the pain.
