Impact of pharmaceutical care on management of uncontrolled hypertension in elderly patients with Diabetes Mellitus type II

Abstract

Despite evidence showing that antihypertensive treatment is effective in the reduction of cardiovascular morbidity and mortality, the rate control of blood pressure (BP) remains inadequate for most affected individuals. This situation could potentially be exacerbated in the presence of associated medical conditions such as Diabetes Mellitus. Thus, this study aims to assess the impact of pharmaceutical care on management of uncontrolled hypertension in elderly patients with diabetes mellitus type II. We'll be conducting a randomized clinical trial in the clinic of Metabolic Diseases, University Hospital, University of São Paulo. This study will include a random sample of 152 patients with a diagnosis of essential hypertension and diabetes mellitus type II, both genders, aged between 60 and 75 years and BP >130/80 mmHg. The patient consultation will consist of a 40-50 minutes face-to-face interview, over 10 months (March to December/2012). The pharmacists' interventions will be focused on health care education and monitoring of drug-related problem. The main outcome measures include: clinical (BP, body mass index, waist circumference, and medication adherence); humanistic (quality of life and patient satisfaction) and economic (use of healthcare resources). It is expected that the pharmaceutical care service can optimize drug therapy, reduce the cardiovascular morbidity and mortality, promoting satisfaction and quality of life for elderly patients with hypertension and diabetes mellitus.
