Local and systemic inflammatory repercussions of pleurodesis in an experimental model

Abstract

Pneumothorax is a disease that admits several therapeutic approaches. The advent of video-assisted surgery helped control the causal factor of pneumothorax in a minimally invasive way. However, doubts remain as to how best perform pleurodesis in pneumothorax, since more aggressive procedures may cause undesirable systemic inflammatory effects. Objective: To evaluate the local and systemic inflammatory effects in experimental pneumothorax in by comparing four methods of pleurodesis habitually used in clinical practice. Method: New Zealand rabbits will be anesthetized, intubated and submitted to selectively videothoracoscopy to perform four types of pleurodesis: apical pleurectomy; abrasion with gauze; abrasion with Marlex mesh, and abrasion with commercially available metal sponge. After 28 days the animals will be euthanized, the chest will be opened to determine the degree of pleurodesis and pleural samples will be taken for microscopic study of fibrosis, collagen deposition and analysis of cytokines in the tissues. In parallel, blood samples will be taken in times pre, immediately post, and after 7, 14 and 28 days after surgery for analysis of leukocytes, LDH and IL-8, VEGF and TGF- ² cytokines. Statistics: ANOVA and Spearman correlation (p <0.05). Expected Results: An unpublished study in the literature, which will be developed in collaboration with Vanderbilt University in Nashville, TN, USA (Prof. Richard W Light), with the support of the Pleural Group of the Pulmonary Division of the University of São Paulo, InCor, FMUSP, which aims to establish the local and systemic effects of different forms of pleurodesis used as an adjunct to surgical treatment of pneumothorax.
