Abstract

Findings from systematic review of the literature showed that children born from (In vitro fertilization) IVF have a higher risk of congenital malformations when compared to those generated by natural conception. The increase in congenital malformations after use of IVF may be related to the hormone treatment used for ovarian stimulation for IVF or the procedure itself, but the factor of infertility and its determinants may also play a role.Among the factors that lead to male infertility indication of the performance of IVF, obstructive azoospermia generated by performing vasectomy issue presents a particular concern about possible birth defects associated with the implementation of IVF.Vasectomy is the most common cause of obstructive azoospermia. The recovery of fertility after vasectomy is obtained by two methods: a vasectomy reversal or IVF with sperm retrieval by epididymal aspiration (PESA). However, the presence of greater number of malformations in the offspring of patients undergoing IVF is a warning that should be done to couples when deciding on the method of recovery of fertility after vasectomy.Among the identifiable genetic alterations, the microdeletion of Y chromosome is among the most studied. The identification of the region of azoospermia factor (AZF) on the long arm of chromosome Y (Yq) led to the development of a key test for the evaluation of serious failures of spermatogenesis. The vast majority of these microdeletions appear de novo, indicating that this region is particularly unstable, and this study leads to the hypothesis that perhaps the AZF region genes may show changes in the procedures of IVF, as it demonstrated to other chromosomes.The incidence of Y chromosome microdeletions of the fertile population is subject of discussion, since the patient's own classification as fertile does not imply normospermia, which in itself can cause bias. While, the incidence of microdeletions in the normal population varies from 0% to 0.87% of cases. The incidence of Y chromosome microdeletions in infertile population studies is highly variable, but is constant the increase of the incidence of these deletions in the population with non-obstructive azoospermia and in those with severe oligozoospermia.In a study by Chung Feng et al. (2008) were observed Y chromosome microdeletions in 10.8% of children born after IVF and no children born through natural conception, the difference being statistically significant. However the sample of the study was small, with 37 cases in the group born from IVF and 60 control group patients born by natural conception, limiting their conclusions.In view of that shown, the presence of greater numbers of Y chromosome microdeletions in the offspring of vasectomized undergoing IVF with sperm retrieval by PESA would increase the incidence of infertility in this population. Thus, the objectives of the study are to compare the incidence of microdeletions on the Y chromosome of individuals born from vasectomies parents before undergoing vasectomy reversal to in vitro fertilization with epididymal sperm retrieved by puncture and to assess the incidence of Y chromosome microdeletions in fertile with normal sperm population.
