Abstract

Sildenafil (20 mg t.i.d.) and tadalafil (40 mg, single daily dose) are oral therapies approved for pulmonary arterial hypertension (PAH). Because there are no comparative studies using these drugs, the present study is aimed at comparing their effects in subjects with PAH associated with congenital heart disease (so called Eisenmenger syndrome). The study will be randomized, and performed over six months. The impact of treatments on the physical capacity will be tested by the six-minute walk test, and pulse oxymetry. Health-related quality of life will be analyzed as well. The possible impact of both phosphodiesterase-5 inhibitors on endothelial and platelet dysfunction will be analyzed by measuring the circulating levels of von Willebrand fact, tissue-type plasminogen activator, Thrombomodulin, P-selectin, beta-thromboglobulin and soluble CD40-L.
