Evaluation of the use of 68Ga-peptide somatostatin analogue PET/CT as a diagnostic tool in neuroendocrine tumors and its correlation with molecular markers

Abstract

Despite recent therapeutic advances, surgical resection remains the only curative treatment for neuroendocrine tumors; therefore the development of more sensitive diagnostic tools that allows earlier detection of disease is highly desirable. The currently used localization studies include ultrasonography (US), computed tomography (CT), magnetic resonance imaging (MRI) and octreoscan; the latter is a functional study based on the property of these tumors to express somatostatin receptors. The high resolution PET/CT with deoxiglucose marked with 18F (18FDG PET/CT) is useful for tumors with aggressive behavior and high glucose consumption. The studies investigating the role of 18FDG PET/CT in neuroendocrine tumors have demonstrated a high sensitivity only with the high-grade tumors, however most of the neuroendocrine tumors have an indolent behavior. The recent development of PET/CT with somatostatin analogs marked with 68Galium allows the association of a high sensitivity structural imaging study with the specificity of a functional study. Recent studies that investigated well-differentiated and moderately-differentiated neuroendocrine tumors have shown the superiority of this tool in comparison to the available imaging studies. The primary goal of this study is to evaluate the efficacy of 68Ga-SOMATOSTATIN ANALOG PET/CT in different types of neuroendocrine tumors, and the secondary goals include the correlation of imaging findings with the proliferative index Ki-67 and with the gene and protein expression of the somatostatin receptors, and the analysis of micro RNAS which are known to regulate expression of these receptors.
