Osteosarcopenic obesity: Crosstalk between osteoporosis, vitamin D, dietary intake and bone fracture risk

Abstract

Introduction: Osteosarcopenic obesity is characterized by a concurrent appearance of obesity in individuals with low bone and muscle mass. It seems to exert a synergistic impact on elderly performance and life quality and expectancy. In the recent years, a plentiful vitamin D extra skeletal benefits has been recognized. Considering it´s interrelation with bone, muscle and fat mass, we hypothesized that vitamin D plays an important role in the etiology of the osteosarcopenic obesity. Aims: to investigate the physiological and genomic mechanisms by which vitamin D could interact with osteosarcopenic obesity in adults and elderly. Methods: This is a cross-sectional study, including adults and elderly individuals from the ISA - Capital study (Inquérito de Saúde do Adulto, a population based study conducted in Sao Paulo). A total of 600 individuals (300 adults and 300 elderly) will be invited to a participate in this arm of the ISA-Capital. The data collection will comprise 5 phases; 1) application of questionnaires: age, sex, ehtina, socio economic characteristics, anthropometrical data, clinical history,dietary intake, sun exposure; 2) blood collection and a second dietary recall; 3) Body composition evaluation by DXA (Dual Energy X-ray Absorptiometry, iDXA GE Healthcare®), walk-and go and handgrip strength tests; 4) Energy expenditure test that will be performed in 51 elderly with the diagnose of obesity, sarcopenia, sarcopenic obesity an osteosarcopenic obesity; 5) biochemical measurements of 25(OH)D, PTH, polymorphisms on genes related to vitamin D metabolism (vitamin D receptor, vitamin D binding protein, and 7 - dehydrocholesterol). The statistical analysis will include mean differences, correlation, and qui-square tests. The regression analysis and general linear model will be used to determine the interplay between vitamin D and body composition variables. The study will be conducted after the approval of the Ethics Committee - FSP - USP. (AU)
