Predicting survival in non-small-cell lung cancer: a link between cancer cachexia, cardiorespiratory fitness and cancer immune surveillance?

Abstract

The present work will translate these findings into patients diagnosed with NSCLC. Our aims are to (A) establish the mechanistic role of our biomarkers in cachexia progression (B) to establish the contributing effects of cachexia and cardiorespiratory fitness on NSCLC prognosis and (C) to examine likely immunological mechanisms that explain why cachexia and low cardiorespiratory fitness are linked to poor clinical outcomes and shorter survival. NSCLC patients (n=20-30, 50% male) will be recruited at the time of diagnosis. Prior to treatment, and following five months of chemotherapy, we will undertake a number of procedures including, but not limited to, collecting a vastus laterals muscle biopsy to investigate mechanisms of cachexia, collecting a blood sample for analysis of inflammatory and immunological variables, and directly measuring cardiorespiratory fitness and muscle function. Aims (B) and (C) are only possible due to the combined expertise of Dr Turner (University of Bath, UK) and Dr Brum (USP, Brazil). Award of this FAPESP/SPRINT application will cement an exciting multidisciplinary international collaboration, allowing mobility of principle investigators, research staff and collaborators, between Bath and USP for data collection, laboratory analysis, undergraduate teaching, and research seminars.
