Abstract

Cirrhotic patients in the liver transplant row are perhaps the group of patients with usual chronic systemic diseases less known and studied in dentistry. The morbimortality of this condition is linked to a huge range of events ranging from infectious processes to hemorrhagic episodes. Dental infections, associated with the immune deficiency present in this condition, are likely to influence the clinical course of liver disease, and may also act as a cause of acute decompensation of cirrhosis (precipitating or aggravating encephalopathy and spontaneous bacterial peritonitis crises). In general, patients with chronic systemic diseases appear to have poorer oral health than the general population, often because of the dentist's fear of managing such patients. In cirrhotic patients, it is likely that this condition is more aggravated by the presence of coagulation alterations, which increase the risk of hemorrhage, increasing the dentist's fear of managing this patient. Knowing the oral health of cirrhotic patients and their impact on the quality of life of the patient will lead to the formulation of action strategies and prevention of oral infections. On the other hand, the extensive evaluation of the coagulation process linked to hepatic physiology and the knowledge of the real risks of bleeding, will bring greater security for the dental surgeon in performing the dental treatment of the cirrhotic patient. The development of this knowledge context has the potential to even positively impact public health actions, since these patients will be able to reach liver transplantation with a much more adequate oral health condition.
