Analysis and modeling of the thermal, optical and mechanical behavior of healthy and demineralized oral tissues during radiotherapy submitted to high-intensity short-pulsed laser irradiation associated or not with bioactive materials

Abstract

Radiotherapy of patients with cancer in the head and neck region causes serious side effects, including radiation-related caries, which is an agressive and severe lesion mainly when affecting root dentin. Considering that the patient is much debilitated during the radiotherapy, a more enduring prevention method is necessary. High-intensity lasers have been shown to be effective in preventing demineralization, but nothing is known about the effects on chemically altered dental hard tissues due to radiotherapy. Nevertheless, the association of lasers with bioactive materials, important for the remineralization of lesions through the formation of carbonated hydroxyapatite, was not explored for these situations. In this project, the objective is to evaluate the effects of near-infrared and ultraviolet emitting short-pulsed lasers in root dentin submitted to ionizing radiation, as well as to verify if the laser radiation can increase the retention of bioactive glass-ceramics. For that, this project is subdivided into four experimental parts: in first one, safe irradiation parameters will be determined for two short-pulsed laser wavelengths (355 nm and 1064 nm, 5 ns) associated with a bioactive glass-ceramic (Biosilicato®), using in vitro tests and a computational finite element model for studyng the generation and propagation of heat. In the other experimental phases, in vitro tests and computational simulation of the mechanical behavior will be conducted and, finally, the anticariogenic action and the mineral loss will be evaluated using a pH-cycling model and microhardness test. Statistical evaluation will be performed, being considered 5% the level of significance. (AU)
