Abstract

According to the American Society of Health System Pharmacists (ASHP), Clinical Pharmacy can be defined as "the science of health whose responsibility is to ensure, through the application of knowledge and services related to the care of the patient, that the use of drugs is safe and appropriate, requiring, therefore, specialized education and structured training in addition to data collection and multidisciplinary interactions". The occurrence of adverse drug reactions (ADR) in hospitalized patients is considered an emerging disease and is currently associated with a significant increase in days of hospitalization, costs, morbidity and possibly mortality of patients, constituting a public health problem due to its high occurrence. The incidence of drug interactions that can generate ADRs in the ICU is higher than the rate of the entire hospital, probably due to the type of medication and the profile of patients admitted to that sector. Clinical Pharmacy in the Intensive Care Unit is already an established practice and has proven to be effective in reducing drug interactions and increasing safety in health services in Europe and the USA. This prospective longitudinal study is based on the evaluation of a certain number of prescriptions and aims to analyze the existence of potential drug interactions in prescriptions in the ICU of the Hospital de Clínicas - Unicamp, classify them, and alert medical staff to avoid them when necessary. It is also intended to evaluate the impact of drug-therapy monitoring by reducing the frequency of occurrence of potential interactions and promoting the rational use of drugs. The study will also help the evaluation of the necessity to develop models of inclusion in Clinical Pharmacy in intensive care units at national public hospitals.
