Evaluation of cephalic and extra-cephalic allodynia associated with temporomandibular disorders

Abstract

Introduction: Temporomandibular disorder (TMD), as well as other musculoskeletal pain, may became chronic with the presence of phenomena such as central sensitization. Allodynia cephalic and extra-cephalic, characterized by reduction in pressure pain threshold (PPT), is a clinical marker for central sensitization. Thus, our hypothesis is that chronic painful TMD is associated with a reduction in the PPT independent of the presence of other painful conditions. Objective: To test whether chronic TMD pain is associated with allodynia cephalic and extra-cephalic. Methods: The sample calculation defined sample of n = 60. Participants will be women who seek dental treatment at Araraquara Dental School, UNESP. Participants will be classified according to the presence of painful TMD [Research Diagnostic Criteria for Temporomandibular Disorders (RDC) - Axis I], grade of depression and somatization (RDC-Axis II), presence of allodynia cephalic and extra-cephalic [ASC-12 / Brazil, PPT with algometer, vibrotactile stimulation with electric toothbrush over the skin] and presence of other painful diseases. To study univariate associations we will apply the Ç ² test and ANOVA (one-way) or Kruskal-Wallis. Multivariate analysis will test the association between chronic painful TMD and allodynia after adjusting for age, depression, somatization, presence of migraine and other chronic musculoskeletal pain. The significance level to be adopted will be 5%.
