Effects of slow and rapid expansion of the maxilla in patients with bilateral and complete cleft lip and palate: evaluation using digital models

Abstract

Introduction: The upper dental arch of patients with complete and bilateral cleft lip and palate commonly presents atretic as a consequence of the absence of the median palatine suture and as a result of the palatoplasty performed at an early age, been necessary, in most cases, realize maxillary expansions with specific appliances aiming to correct the inadequate form of the upper arch. Objective: To evaluate and compare the occlusal effects of the maxillary expanders Hyrax and quad-helix in patients with complete bilateral cleft lip and palate. Materials and methods: The study sample will consist of 60 patients with complete and bilateral cleft lip and palate associated to an atresia of the upper dental arch. The patients will be randomly and prospectively divided in two groups, one treated with the Hyrax expander (n = 30) and other with quad-helix expanders (n = 30). Conventional plaster casts will be acquired immediately pre-expansion and 6 months after the expansion at the time of the removal of the appliances. After scanning the plaster models of the upper arch using the 3Shape R700 3D scanner (3Shape A / S, Copenhagen, Denmark), the transverse dimensions of the dental arch, the labial inclination of the posterior teeth, the arch perimeter and its transverse dimensions, and the volume of the palate will be measured. The interphase changes will be evaluated using the Student "t" test (p <0.05) and the comparison of the initial characteristics of each group, as well as the intergroup comparison of the effect of the expansion, will be performed by independent t test (p <0.05).
