Abstract

The purpose of this project is to study temporomandibular joint (TMJ) osteoarthritis and its relationship with potential predisposing factors and/or enhancers by new computational techniques developed to optimize the use of three-dimensional images. It will be evaluated Cone Beam Computed Tomography (CBCT) scans of 100 patients (50 women and 50 men) who underwent mandibular advancement surgery with counterclockwise rotation of the maxillomandibular complex, obtained at the following intervals: T1, presurgery; T2, immediately after surgery; and T3, at least 11 months after surgery. The sample will be divided into 2 groups: Group 1, patients with articular disc displacement (DD) who underwent articular disc repositioning surgery; Group 2, patients without DD whom, therefore, did not undergo TMJ surgical intervention. Firstly, it will be tested the validity of a new method for three-dimensional assessment of the mandibular condyles, which proposes voxel-based regional registration and shape correspondence measurements. The proposed method will be applied for assessing condylar changes after mandibular advancement surgery with counterclockwise rotation of the maxillomandibular complex, with or without TMJ surgical intervention. Statistical analysis will be conducted to test the reproducibility of the method; to quantitatively evaluate condylar remodeling in each group; to identify differences between the groups; and the degree of correlation between the magnitude of surgical displacement and condylar torque in T2 with osteoarthritis severity in T3. The validation and application of the proposed methodology may be useful for diagnosis, risk factors identification, planning, prognosis prediction and surgical outcomes analysis.
