Abstract

The activity of Bothrops jararaca venom is broad and complex on biologic systems of preys, showing pro- and anti-hemostatic features. Venom proteins may alter by various approaches the hemostatic systems. Several systemic manifestations are described during B. jararaca envenomation, and bleedings are the most conspicuous one. Systemic bleeding is multifactorial, and patients manifest consumption coagulopathy with specific particularities that have not been completely elucidated. Restoration of hemostatic disorder occurs progressively after administration of specific antivenom, which neutralizes circulating venom. However, stimulatory mechanisms for this recovery are still unknown. Information about the kinetics of hepatic synthesis of clotting factors may assist on the treatment of patients. On the other hand, thrombocytopenia and platelet disfunction, as well as the presence of mucous bleeding, which shares clinical similarity with symptons of von Willebrand disease, are also commonly observed in Viperidae envenomation. Thrombotic microangiopathy has been reported to following snakebites, and the occurrence of intravascular hemolysis has been associated with this disease. Von Willebrand factor (vWF) and its cleavage enzyme, ADAMTS13, could be closely related to this process, such as it occurs in cases of thrombotic thrombocytopenic purpura, where a reduction of ADAMTS13 activity and platelet microthrombi are found, associated with thrombocytopenia and microngiopathic hemolytic anemia. Therefore, we aimed in this study: (1) to analyze the occurrence of quantitative and qualitative disturbances in von Willebrand factor (vWF) and its cleavage enzyme, ADAMTS13, during experimental B. jararaca envenomation; (2) to investigate the synthesis of coagulation factors during envenomation.
