The natural history of the cerebral blood flow regulation after acute ischaemic stroke

Abstract

Acute stroke is known to lead to impairment of cerebral blood flow (CBF) regulation. Experimental and clinical studies have suggested that neuronal and cerebral hemodynamic reorganization is one of the main mechanisms responsible for brain recovery. However, the lack of understanding of crucial points limits the translation of the knowledge into clinical practice. This study aims to assess the main CBF regulatory mechanisms (cerebral autoregulation, CA and neurovascular coupling, NVC) over 3-months after acute ischaemic stroke. Stroke participants will be recruited after a first ever episode of ischaemic stroke within 24 hours of symptoms onset. Age- and blood pressure-matched controls free from pre-existing neurological disorders will be also recruited. Recording of CBF velocity using transcranial Doppler ultrasound, beat-to-beat blood pressure and end-tidal CO2 will be performed during 5 min baseline (to assess CA) and 1 min passive movement of the elbow (to assess NVC). Stroke participants will be assessed < 24 hours of the stroke on set, and at 2 weeks, 1 and 3 months post-stroke. We believe our study will show an important impairment of both regulatory mechanisms initially following the acute onset, and return to control levels during the recovery period. These findings are relevant to guide the timing of interventions to manipulate BP, and potentially for the impact of intensive rehabilitation strategies that may precipitate acute physiological perturbations.
