Immediate effect of a pilates method training session on trunk stabilizer muscles co-contraction pattern in individuals with and without nonspecific low back pain

Abstract

Previous studies demonstrated that the insufficiency on muscular function leads to stress and undue loads on vertebral spine joints and ligaments. Currently, the exercises of segmentar lumbo-pelvic stabilization are considered, in physical therapy, the ideal therapy to prevent and rehabilitee nonspecific low back pain. Among this class of exercises is the Pilates Method. The execution of exercises of Pilates Method aims to voluntary to recruit the trunk stabilizer muscles with the objective to promote a new neuromuscular program, which may be transferred to activities of daily living. Thus, it is important to determine if the practice of Pilates Method can promote changes on trunk stabilizer muscles cocontraction pattern. Objective: The present study aims to analyze the immediate effect of one session of exercises of Pilates Method on trunk muscles cocontraction pattern in subjects with and without nonspecific low back pain. Methods: Adult subjectsof both genders will participate of this study. These subjects will be separate into two different groups: low back pain group and group without low back pain. The procedures will be performed in two days separated by an interval of 24-36 hours. In the first day, the personal data, which permitted an identification of the subjects will be collected. After that, the maximum isometric voluntary contraction (MIVC), the Biering-Sorensen test and the familiarization to the Pilates exercises will be performed. In the second day, the test of arm elevation will be performed and, then the Pilates Method exercises protocol will be performed after that the Biering-Sorensen test and the arm elevation test will be re-assessed. To determine the MIVC will be performed by four tests in different positions and each test will be performed three times by four seconds. To perform the Biering-Sorensen, the participants will be positioned lying prone on an assessment be, with iliac spine positioned on the borderline of the bed and the trunk on a removable surface. The arms will be cross in front of the chest and the participants will be oriented to move the trunk until the maximum amplitude of extension and, then, they will have to maintain the isometric contraction of the spine erector muscles at this position. A chronometer will record the time and the test will finish when the subjects cannot maintain this position. The arm elevation test will be performed with an upper limb flexion. During this test will be performed 10 movements of shoulder flexion until 180º of flexion. Three exercises of Pilates Method (Hundreds I, One Leg Stretch level I e One Leg Circle level I), which exercise the primary muscles chain, will be performed. The exercise protocol will have approximately 30 minutes. The EMG signal of internal oblique (IO), multifidus (MU), iliocostalis lumborum (IL), upper rectus abdominis (URA) and anterior deltoid (AD) will be recorded. EMG signal will be rectified (full wave rectification, filtered (band pass filter 20-500 Hz) and smoothed (low pass filter of 4th order and cutoff frequency of 6 Hz) to the creation of linear envelope. The linear envelope will be normalized by the MIVC contractions and then the index of cocontraction calculation will be performed. Therefore, the EMG onset time will be determined by the change of two standard deviation since the value of rest activation and the difference between the onset time of AD and IO and AD and MU will be calculated.
