Impact of atraumatic restorative treatment associated with oral health educational strategies on dental anxiety of schoolchildren

Abstract

The objective of this study will be to evaluate the impact of Atraumatic Restorative Treatment (ART) associated with two oral health educational approaches on dental anxiety of school children, in terms of self perception, anxious behavior, salivary cortisol and alpha-amylase levels and Heart Rate (HR). 135 children will be selected, aged six and seven years old, both genders, from public elementary school in Saltinho and Charqueada, São Paulo, who present at least one deciduous molar affected by dentine caries lesion. Children will be grouped in three groups according to the application or not of educational strategies before ART procedures: traditional strategy + ART (G1=45), enhanced strategy + ART (G2=45) and ART (G3=45). The traditional strategy consists of an explanation of caries etiological factors, oral health orientation and practices during four weeks, once a week. The enhanced strategy consists of the same period and content of traditional strategy in addition to caries lesion self-visualization with mirror and orientation about ART´s importance, indication and stages. Self perception of dental anxiety will be assessed by the modified version of Venham Picture Test (VPT-m) in three moments: before educational strategy, before and after ART. The anxious behavior will be assessed by the modified Venham Anxiety Scale (VAS-m) in four moments: during explanation, excavation, insertion of material and after ART. Salivettes will be used to collect saliva for cortisol and alpha-amylase dosage in four moments: before the educational strategy (baseline), 10 minutes before ART (T0), 25 (T1) and 35 minutes (T2) after ART. Cortisol and alpha-amylase levels will be calculated using enzyme immunoassay kits and optical density analysis by ELISA. The maximum and average HR will be evaluated using a cardiac monitor in six moments: before educational strategy (baseline), 10 minutes before ART, during explanation, excavation, insertion of material and after ART. The description of the studied variables and their correlations will be analyzed by descriptive statistical analysis, normality test, parametric and non-parametric tests, where appropriated.
