Abstract

Bone fragility was recently recognized as one of the complications of type 2 diabetes mellitus (DM2). Bone mass in DM2 is normal or elevated when compared with a control group, and bone fragitily may be related with impairments in bone quality. Bone densitometry, even considered as a golden standard in the evaluation of bone density, does not reveal parameters related with bone quality. Thus, there is a need to explore other tools that could unveil such measeurements. Magnetic resonance imaging (MRI) is a technique capable of quantifying bone marrow adipose tissue (MAT) and apparent trabecular volume (appTV). MAT is considered as an important parameter related negatively with with bone mass, for instance, in anorexia nervosa and osteoporosis. Given that bone fragility in DM2 is a consequence which takes to increase of morbidity and mortality, it is important to identify risk groups in order to prevent fractures. To that aim, exams like lumbar spine and muscle MRI spectroscopy, bone marrow adiposity measurements by spectroscopy, and MRI trabecular imaging in proximal tibia will be accomplished to elucidade the role of body fat distribution in bone mass parameters. Bone densitometry will be carried out to quantify bone mass and also body composition of patients and controls. Biochemical exams will identify factor which may intervene in the fat overflow from adipose tissue into muscle and bone marrow. Also, these exams will be useful to identify alteration of secreted factors by these same systems. (AU)
