Prevalence and factors associated with pre-diabetes, undiagnosed diabetes and diabetes in people over 50 years of age

Abstract

Individuals with undiagnosed and, therefore, untreated diabetes are at increased risk of disease-related comorbidities and mortality. Furthermore, with the worldwide increase in the prevalence of obesity, surveillance of pre-diabetes and undiagnosed diabetes, as well as knowledge of its associated factors are crucial to understanding the extent of the problem and for health policies to be implemented. Thus, the present study aims to examine the prevalence and factors associated with pre-diabetes, undiagnosed diabetes and diagnosed diabetes in Brazilians and English people over 50 years of age. This is a cross-sectional study with data from 9,169 participants of the English Longitudinal Study of Aging (ELSA) in 2012, and 9,412 participants in the Longitudinal Study of Health and Well-being of Brazilian Elderly People (ELSI-Brazil) in 2015. The Diabetes classification will be performed through self-report of diabetes and glycated hemoglobin values following the recommendation of the American Diabetes Association. Participants will be divided into four groups: non-diabetic, pre-diabetic, diagnosed diabetics, and undiagnosed diabetics. The prevalence of diabetes conditions will be calculated with a 95% confidence interval. Multinomial regression models adjusted for factors known to be associated with diagnosed diabetes will be adopted to analyze factors associated with pre-diabetes, undiagnosed diabetes and diagnosed diabetes, with reference to the non-diabetic category. The hypothesis of the present study is that pre-diabetes and undiagnosed diabetes have a high prevalence in the population over 50 years of age and that the associated factors are different between England and Brazil and, due to socioeconomic and behavioral conditions, the prevalence is higher in the latter.
