Effects of frailty and C-terminal Agrin fragment as prognostic biomarkes in septic shock patients

Abstract

Septic shock has high global prevalence, and is considered the most important cause of death in critically ill patients. Among survivors, the poor quality of life, the development of frailty and increased mortality are major concerns. Frailty is characterized as physical and cognitive impairment that leads to worse outcomes. Several frailty scores were validated and used during hospital stay, including the Clinical Frailty Score (CFS). In addition, other biomarkers are been used in the identification of muscle depletion and frailty, such as C-terminal agrin fragment (CAF). However, there is no study that evaluates the effects of frailty and CAF, in the prognosis of septic shock patients. Thus, the objective of this study is to evaluate the association between frailty and serum levels of CAF with length of ICU stay and mortality in patients with septic shock. Patients over the age of 18 years, of both sexes, with septic shock admitted to ICU will be prospectively evaluated. We plan to include 92 patients. At ICU admission demographic, clinical and laboratorial data will be recorded. Within 24 hours of ICU admission, blood sample will be collected to measure CAF, and the CFS will be applied. The length of ICU stay and ICU mortality will be recorded. We will perform Student t test or Mann-Whitney according to variables distribution. For categorical variables we will use Chi-square or Exact Fisher test. Multiple linear and logistic regression will also be performed. The significance level will be 5%. (AU)
