Articulation of care for drug-using children and adolescents: case study in a foster care unit

Abstract

Public policies aimed at protecting and caring for children and adolescents with drug-related problems are recent and poorly structured in Brazil. In recent years, we have seen an increase in hospitalizations of drug users, especially young people. Childcare and Youth Units (UAi) are modalities mentioned by the current legislation as options of care for this population. However, there is a shortage of studies in these institutions. Thus, this study aims to understand how a UAi in the interior of São Paulo has been articulating the care for children and adolescents with needs arising from drug use in situations of social and family vulnerability. In addition to analyzing the situations of social and family vulnerability of children and adolescents drug users hosted in an UAi; understand, from the theoretical framework of mothering, how an UAi has been providing care to children and adolescents with needs arising from drug use in socially vulnerable situations; and describe characteristics related to the revolving door phenomenon in an UAi and analyze how the service handles this phenomenon. In order to achieve these objectives, a qualitative case study was conducted, using data collection, 65 hours of participant observation recorded in a field notebook, documentary analysis, semi-structured interview with eight professionals from UAi and focus group with six professionals from Children\'s Psychosocial Care Center - Alcohol and Drugs (CAPSiAD). From the data analysis were built three broad themes called social vulnerabilities, maternity and revolving door, divided into three manuscripts. These themes address: 1) The analysis of social vulnerabilities experienced by the host who deal with: social exclusion, situations of violence, conflict with the law, homelessness and family relationship; 2) The importance of a structured and welcoming environment, as well as a professional and empathetic look of professionals to enable the broadening of perspectives of adolescents and the construction of new ways of relating, through dialogue and the management of aggression; 3) The factors that contribute to the occurrence of the revolving door phenomenon such as lack of family bond and adhesion, public underfunding and absence of public policies. Children and adolescents who are drug users are still served by adult devices, so thinking about ways of caring for this population is essential to obtain more subsidies that enable the formulation and construction of specific public policies. Drug use is somewhat cross-sectional to the vulnerability situations discussed, as these are adolescents who have been sheltered due to drug use, but their treatment should be planned covering broader social issues. Otherwise, care may end up reversed in new forms of discrimination and social exclusion, whether caused by institutionalization, medicalization, social control, criminalization and pathologization of poverty. The family needs to be prepared and instructed by professionals to understand the service proposals and to recognize that they are part of the care process offered by the institutions. Important actions are aimed at greater funding for the implementation of services specifically developed for this population, as well as professional qualification and strengthening of the network for the articulation of care to be effective.
