Cities of rights: the healthcare policies for international migrants in the cities of São Paulo (Brazil) and Barcelona (Spain)

Abstract

Local governments have claimed and played an increasingly relevant role in the development of policies for international migrants. Barcelona, in Spain, and São Paulo, in Brazil, stand out in their respective regional contexts as cities that have implemented policies for migrant populations in their territories, oriented towards protecting their rights. Considering them from the perspective of global health, this study aimed to assess how these policies are implemented on the ground, in light of federative relations, national migration policies, and the participation of international entities in migration governance. For this, the study relied on extensive document research and semi-structured interviews with key local actors, focusing on the inclusion/exclusion of migrants in public health services in both cities. It was observed that, in spite of local bureaucratic efforts, in both cases a series of barriers undermining the quality of migrant healthcare persist, due to deliberate policies of healthcare exclusion (in the Spanish case), as well as neglect and resource shortages (in the Brazilian case), which harms the principles of universality and equity underpinning their national public health systems. We conclude that the local dimension is an important, but insufficient, realm for the implementation of migration policies and for migrants\' right to health, as it suffers from a fragmented and uncoordinated political pattern with other spheres of government.
