A randomized, placebo-controlled study evaluating the hemodynamic and metabolic effects of Methylene blue treatment in septic shock

Abstract

Septic shock is the most common cause of death in intensive care unit worldwide and early therapy has been proven to reduce mortality. Despite some recent advances in diagnosis, monitoring and therapy, the mortality rate remains high. It is a life-threatening organ dysfunction due to a dysregulated host response to infection. It is characterized by arterial vasodilation, increased capillary permeability and systemic tissue hypoperfusion, which if not treated rapidly can evolve to multiple organ failure and death. Some review studies, pilot studies and case reports have already shown benefit of the association of methylene blue as a coadjuvant agent for the treatment of vasodilation resulting from septic shock. However, most of these studies were not performed within the first few hours of septic shock. The aim of this study is to determine if the administration of methylene blue in patients in the early stage of septic shock leads to a resolution of shock compared to conventional treatment. This is a randomized placebo-controlled trial with a sample calculation that will show differences in the resolution of septic shock between groups of 30%. (AU)
