Effectiveness of aerobic training in chemotherapy-related adverse symptoms during treatment: controlled and randomized trial with economic evaluation

Abstract

One of the strategies to prevent adverse effects arising from chemotherapy treatment is to perform physical exercises during the duration of the treatment. However, there is still no consensus on the best type and intensity of exercise, nor on when it should be started. Most studies have been carried out in patients with breast cancer, usually a few weeks after starting chemotherapy, on an outpatient basis, two to three times a week. The main differentials of our study are: the execution of physical training in hospitalized patients for the completion of the chemotherapy cycle for cancer treatment, this training being carried out five times a week and not being restricted to a specific type of cancer. Objectives: To evaluate the effects of aerobic training on chemotherapy-related symptoms (nausea, vomiting, asthenia and feeling of weakness), fatigue, mobility, clinical complications and length of hospitalization of patients during the cycle of drug treatment, adverse effects of training and the implementation of the proposed intervention, in addition to evaluating the cost-effectiveness of this intervention. Methods: Controlled and randomized trial, with blind evaluation that will include 94 cancer patients who are hospitalized for one or more cycles of chemotherapy. The intervention group will perform aerobic training during the duration of a cycle of chemotherapy. The control group will receive a booklet with guidelines to stay active during the hospitalization period. The groups will be compared using the Mixed Linear Model, regarding fatigue and mobility before and after the intervention. The length of hospital stay will also be compared between groups by Kaplan-Meier analysis of survival. The incidence of chemotherapy-related symptoms and complications will be compared using the chi-square test. Cost-effectiveness and cost-utility analyzes will be performed for the impact of exercise and quality-adjusted life years (QALYs) using the EQ-5D-3L21 quality-of-life trials.
