Evaluation of metabolic syndrome in patients with primary antiphospholipid syndrome

Abstract

The antiphospholipid syndrome is characterized by thrombosis and recurrent miscarriage and is considered the most common acquired thrombophilia. When it isn't associated with any connective tissue disease, it is so-called primary and its treatment is based on long-term anticoagulation with warfarin. It's an autoimmune disease associated with high cardiovascular risk and it's believed that antiphospholipid antibodies are able to induce endothelial dysfunction, influencing the vasodilatation and acting as the trigger to accelerated atherosclerosis that is observed in many patients. The metabolic syndrome is characterized by a state of insulin resistance, impaired glucose tolerance, hyperinsulinemia, elevated triglycerides, reduced high-density lipoprotein cholesterol and hypertension. Questioned whether patients with APS and MS would have greater cardiovascular risk, because they have other factors that are known to predispose the cardiovascular disease such as high blood pressure, dyslipidemia, hyperglycemia and obesity associated with the fact that both diseases have similarities in their pathogenesis about the endothelial dysfunction. Despite admitting the importance of MS as cardiovascular risk factor in the general population and in patients with rheumatic diseases in particular, little is known about its prevalence and its risk in patients with primary APS, so far, we did not find any article in the international literature on this association. The purpose of this study is to assess the prevalence of MS in patients with primary APS, compared with a population of healthy controls; evaluate MS's relation with other cardiovascular risk factors and inflammation.
