Abstract
Chronic renal failure is associated with disturbances in mineral homeostasis due to bone abnormalities related to a positive calcium and phosphorus balance, resulting in higher risk of fractures, vascular calcification and high mortality. In clinical practice, little attention is given to the dialysate composition. Our group recently showed that bone remodeling could affect calcium mass transfer during hemodialysis. However, bone remodeling was then estimated by PTH and osteocalcin. The aim of this study is to analyze the influence of bone remodeling on calcium mass transfer during hemodialysis in patients with hyperparathyroidism pre and post parathyroidectomy. A total of fifteen patients will be selected and all of them will be submitted to a bone biopsy. Total bone osteocalcin will be estimated and biochemical markers of bone remodeling will be measured. Patients will will be submitted to hemodialysis sessions with dialysate calcium of 1.25; 1.5;1.75 mmol/L before parathyroidectomy. Later on, the protocol will be repeated and we will use their previous results as controls. (AU)
