Abstract

The mastication function of complete denture wearers has been investigated. In this context, posterior tooth arrangement and occlusal form applied to the design of complete dentures are of primary importance. Clinical and in vitro studies evaluating posterior occlusal schemes used in complete denture fabrication have been conducted. There remains a lack of conclusive evidence for recommending one occlusal scheme for complete dentures over another. As a consequence, the decision about the occlusal scheme to be used in complete denture fabrication is attributed primarily to the clinician's personal preference and experience One of the goals in the treatment of edentulous patients is to achieve acceptable masticatory function, but overcoming the inherent limitations of complete dentures is a challenge. Two popular occlusal schemes of occlusion for complete denture constructions are conventional balanced occlusion (CBO) and lingualized balanced occlusion (LBO).As a consequence of the lack of conclusive evidence for recommending one occlusal scheme for complete dentures over another the Fapesp Project # 2007/02240-8 (sept/2007 to aug/2008) was developed by us, resulting the article "A randomized crossover clinical trial comparing masticatory function between lingualized balanced occlusion and conventional balanced occlusion for complete dentures", accepted for publication in the Journal of Prosthetic Dentistry (ISSN 0022-3913). This study demonstrated a statistical difference in the masticatory efficience between the studied occlusal schemes, with better results for the Lingualized Balanced Occlusion when toasted coffee beans were utilized as a test food. A statistical difference was not observed when almonds were utilized. This was justified by the fact that the number of strokes was not enough to properly chew the almonds.The purposes of this project are: (1) to develop a randomized crossover clinical trial comparing masticatory function between lingualized balanced occlusion and conventional balanced occlusion for complete dentures using almonds as a test food, masticated with (a) 30 strokes, (b) 40 strokes and, (c) until the subject to feel read to swallow the bolus and; (2) to develop a randomized crossover clinical trial comparing the oral healt self-perception(GOHAI)of wearers of complete dentures fabricated with conventional balanced occlusion and with lingualized balanced occlusion. In the studies (1) and (2) 30 edentulous subjects will receive complete dentures with interchangeable posterior lower teeth that will allow to change the occlusal scheme without to change the base of the lower prosthetic appliance. Previously to the study number (2) a validation of the questionary (GOHAY) in Portuguese language will be developed. The sample of the study #2 will be of 100 edentulous subjects, weares of maxillary and mandibular complete dentures. (AU)
