Abstract

The extracranial carotid atherosclerotic cerebrovascular disease is an important cause of stroke and is characterized by thickening of the intima-media and presence of plaque in the vascular wall. Since baroreceptors are located in the adventitia of the carotid bulb and the right subclavian artery, decreased stretch caused by the presence of atherosclerotic plaque in these locations could lead to reduced baroreflex sensitivity, and thus be reflected in heart rate variability (VFC). We have therefore hypothesized that the carotid and subclavian atherosclerotic lesions might be associated with alteration of the autonomic nervous system (ANS) considering that the ANS can be assessed by heart rate variability (HRV). Objectives: * To investigate possible changes in the autonomic nervous system through analysis of heart rate variability in atherosclerotic disease that affects the extracranial carotid arteries and the right subclavian artery. * Identify a parameter / index of heart rate variability reflecting the activity of the autonomic nervous system that correlates with the degree of carotid atherosclerotic lesion and right subclavian, which could allow an early diagnosis of disease. Methods: This will be a transversal study involving asymptomatic patients aged e 18 years. To determine autonomic activity in atherosclerotic extracranial carotid and right subclavian, we propose to evaluate the HRV via ECG parameters and vascular duplex ultrasonography. The HRV will be evaluated under normal daily locomotor activities and under tilt-test. The proposal is to use statistical tests or analysis of variance comparison between groups used in accordance with the distribution of the data. Expected results and significance of the project: We hope the results of this proposed project will provide us with advancement in early diagnosis of extracranial atherosclerotic cerebrovascular disease.
