Evaluation of antimicrobial photodynamic therapy as an adjunct to surgical therapy access scaling in patients with advanced chronic periodontitis

Abstract

Chronic periodontitis (CP) is an infectious disease resulting in inflammation of the supporting structures of the teeth, progressive loss of bone loss and insertion. The aim of this study is to evaluate the effects of antimicrobial Photodynamic Therapy (aPDT) as adjuvant to periodontal surgery treatment (PST) in patients diagnosed with generalized severe chronic periodontitis (GSCP). In a split mouth design, 20 patients with clinical diagnosis of GSCP will will receive scaling and root planing (SRP). After that, the sites indicated for surgical therapy will be treated with full thicknes flap associated + SRP, associated or not to aPDT. aPDT will be performed by using a laser light source with 660 nm wavelength associated with a phenothiazine photosensitizer. All patients will be monitored for 90 days. Clinical assessment of plaque index, probing depth, clinical attachment level and bleeding on probing will be performed at baseline (pre-intervention period) and 30 and 90 days after surgery. Subgingival plaque samples will be collected at baseline and 30 and 90 days after surgery and the counts 40 subgingival species will be determined using the method of checkerboard DNA-DNA hybridization. Gingival crevicular fluid samples will be collected at baseline and 30 and 90 days after the surgery for evaluation of the volume of fluid (Periotron) and the levels of interleukin 1 beta, MMP-8, tumor necrosis factor alpha (Luminex), RANK-L and Osteoprotegerin (OPG). The data will be statistically analyzed with the appropriate tests for the sample. (AU)
