Evaluation of omega-3 polyunsaturated fatty acids plus low-dose aspirin daily supplementation to treat Generalized Aggressive Periodontitis: randomized controlled clinical trials

Abstract

Periodontitis is an inflammation that leads to destruction of gingival and periodontal tissues. Among the forms of periodontal disease, Aggressive Periodontitis (AgP) has the fastest rate of progression and affects young and systemically healthy individuals. However, although studies show that the association of an antimicrobial with mechanical therapy promotes better results, the large-scale use of antibiotic agents may generate bacterial resistance in the periodontal microbiota. Therefore, other forms of therapy need to be developed. In patients with AgP, the interindividual variation in response to therapy tends to be generalized. Thus, research-involving modulation of host inflammatory responses may be of great value to these patients. Recently, the daily supplementation of omega-3 polyunsaturated fatty acids and aspirin was proposed as adjunctive therapy for the treatment of chronic periodontitis, demonstrating good clinical results. Aspirin showed good results by inducing the production of lipoxins triggered by aspirin, which are more bioactive than common lipoxins and have better inflammatory resolution properties. Studies have shown that omega-3 polyunsaturated fatty acids serve as substrate for the enzymatic conversion of lipidic mediators called resolvins and protectins, which present great potential for protective and pro-resolutive actions in inflammatory diseases, such as Periodontitis. Thus, the aim of this randomized controlled clinical trial of superiority will be to evaluate the effect of 3 g of omega-3 polyunsaturated fatty acids and 100 mg of aspirin daily supplementation over a period of 180 days as adjunct to non-surgical and surgical therapies of patients with Generalized Aggressive Periodontitis. Probing depth, clinical attachment level, gingival index and concentration of microorganisms and cytokines at baseline, 3, 6 and 12 months after the procedure will be evaluated. (AU)
